A 40-year-old roofing contractor from Rockland County, NY, with a history of safety violations and two employee deaths, was charged with knowingly and willfully violating federal safety regulations, including failing to protect his employees from fall hazards, resulting in injuries and deaths.
The penalties are in addition to a proposed $633,500 in OSHA penalties – still mostly unpaid – for similar violations found during 16 inspections since 2004.

OSHA previously cited the contractor for fall-related violations at worksites in Boston, Medford, Haverhill and Dorchester, Massachusetts between 2014 and 2016.
